Using Medicaid for therapy

Medicaid coverage for mental-health varies significantly by state — each state administers its own Medicaid program within federal guidelines, and what's covered, how it's covered, and which therapists accept it differs accordingly. Most state Medicaid programs cover outpatient mental-health therapy with low or no copays, but the network of providers willing to accept Medicaid rates is often limited compared to commercial insurance.

Federal Medicaid requirements include coverage for "rehabilitative services" which includes mental-health, but the implementation details vary. Most states cover individual therapy, group therapy, and psychiatry. Most cover telehealth (this expanded significantly during and after 2020 and the expansion has largely become permanent). Some states require referrals; some cover specific evidence-based therapies (DBT, EMDR, TF-CBT) and not others.

For people enrolled in a Medicaid managed-care organization (MCO) — most Medicaid members nationally are now in MCOs rather than traditional fee-for-service Medicaid — your therapy benefits are administered through the MCO's network. Common MCO administrators include Magellan, Beacon/Carelon, and state-specific organizations. Your card will name the MCO; the therapist needs to be in-network with that specific MCO, not just "Medicaid" generally.

Whether you should expect to find a Medicaid therapist quickly depends heavily on your state and region. Some areas have strong Medicaid mental-health networks; others have severe shortages. If finding an in-network Medicaid therapist is taking weeks or months, some additional options to consider:

Community mental-health centers and Federally Qualified Health Centers (FQHCs) accept Medicaid and use sliding-scale fees for uninsured clients. They're often the most reliable access point for Medicaid mental-health care.

Some private-practice therapists offer sliding-scale fees for clients with Medicaid even when they're not in-network, sometimes through limited-capacity scholarship programs. The matching form lets you note that affordability matters.

If you're enrolled in Medicaid and your specific MCO won't authorize timely access to mental-health care, your state's Medicaid ombudsman can help — federal parity rules apply to Medicaid managed-care plans the same way they apply to commercial coverage.

To find a Medicaid-accepting therapist, submit the matching form. Our intake coordinator can help you identify which MCO administers your benefits and which therapists in your area are in-network.
